Referral of information to Aged Care Quality and Safety Commission template

This template is used to refer key information, concerns or positive feedback to the Aged Care Quality and Safety Commission about care and services provided by Residential Aged Care Providers and Home Care Service Providers. 

Once completed this document should be sent to info@agedcarequality.gov.au using the service name and state in the email subject heading. This is the preferred method of referral of information from allied health/health professionals such as GPs, paramedics, physiotherapists, podiatrists, hospital staff etc and any other Commonwealth/State/Territory government, body, regulator or agency.
