Following a site audit conducted on 29 March 2022 to 01 April 2022, the Commission made a decision on 05 May 2022 to re-accredit this service.

The period of accreditation of the service will expire on 13 August 2025.

Following a site audit conducted on the 25 February 2020 to 27 February 2020, the Commission made a decision 07 April 2020 to re-accredit this service.

The decision on the service’s accreditation period was varied following reconsideration on own initiative on 23 March 2022.

The period of accreditation of the service will expire on 13 August 2022.

The period of accreditation of the service will expire on 13 May 2022.

Compliance monitoring update

An assessment contact was conducted by the Aged Care Quality and Safety Commission (Commission) on 15 May 2019 at Forest Lake Lodge to monitor the service’s progress in meeting the Accreditation Standards. The Commission found the service complies with all Accreditation Standards.

Following an audit we decided that this home met 42 of the 44 expected outcomes of the Accreditation Standards and would be accredited for one year until 13 May 2020.

Serious risk decision

On 26 March 2019 a delegate of the Aged Care Quality and Safety Commissioner made a decision in relation to Forest Lake Lodge that failure to meet one expected outcome in the Accreditation Standards placed the safety, health or wellbeing of a consumer of the service at serious risk.

An assessment contact was conducted on 15 May 2019 at Forest Lake Lodge to monitor the service’s progress.  The Aged Care Quality and Safety Commission found the service complies with all Accreditation Standards.

Serious risk is not an ongoing state; it is a statutory decision based on evidence at a point in time.

Following an audit we decided that this home met 44 of the 44 expected outcomes of the Accreditation Standards and would be accredited for one year until 13 May 2019.

Serious risk decision

On 13 November 2017 a delegate of the CEO of the Australian Aged Care Quality Agency made a decision that Lollies Management Pty Ltd failed to meet one of more expected outcomes in the Accreditation Standards in relation to Forest Lake Lodge and that this failure has placed, or may place, the safety, health or wellbeing of a care recipient of the service at serious risk.

The Department of Health has been notified of the risk.

The Quality Agency will continue to monitor the performance of the service including through unannounced visits.

Following the audit, an authorised delegate of the CEO of the Australian Aged Care Quality Agency made a decision on 13 November 2017 to re-accredit this service with an accreditation expiry date of 13 May 2018. 

The approved provider of the service meets 39 of the 44 expected outcomes of the Accreditation Standards.   The audit report is attached.

Following an audit we decided that this home met 44 of the 44 expected outcomes of the Accreditation Standards and would be accredited for three years until 12 October 2018.
