Exceptional Circumstances

During the COVID-19 pandemic, the Aged Care Quality and Safety Commission has temporarily modified our Regulatory Program, including the suspension of site audits to determine whether to accredit, not to re-accredit or vary the periods of accreditation for a service. 

In order to give effect to continuity of accreditation the Department of Health has, under Section 42.5 of the Aged Care Act 1997, made a decision to grant ‘exceptional circumstances’ to this service until 07 August 2021.

The service remains subject to compliance monitoring by the Aged Care Quality and Safety Commission.

The period of accreditation of the service will expire on 7 August 2020.

Following a site audit the Commission made a decision on 18 October 2019 that this service met three of the eight Aged Care Quality Standards. The Commission decided not to re-accredit the service. The service remains accredited until 18 October 2019.

Serious risk decision

Following a re-accreditation audit conducted on 9 to 16 September 2019 a delegate of the Aged Care Quality and Safety Commissioner made a finding that Bupa Aged Care Australia Pty Ltd failed to meet one Aged Care Quality Standard (Quality Standards) in relation to Bupa Seaforth.

On 17 October 2019 a decision was made that the failure to meet one Quality Standard has placed the safety, health or well-being of aged care consumers of the service at serious risk. The Department of Health has been notified of the risk.

Serious risk is not an ongoing state; it is a statutory decision based on evidence at a point in time.

 

Following an audit we decided that this service met 18 of 44 expected outcomes of the Accreditation Standards. The audit was conducted on 29 April 2019 to 02 May 2019. The assessment team's report is attached.

A delegate of the Aged Care Quality and Safety Commissioner made a decision on 10 June 2019 to revoke accreditation of this service. The period of accreditation of the service will expire on 17 October 2019.

Serious risk decision

Following a review audit conducted on 29 April – 02 May 2019, a delegate of the Aged Care Quality and Safety Commissioner made a decision that Bupa Aged Care Pty. Ltd. failed to meet 26 expected outcomes in the Accreditation Standards in relation to Bupa Seaforth.

On 18 June 2019 a decision was made that failure to meet six of these expected outcomes has placed the safety, health and wellbeing of consumers of the service at serious risk. The Department of Health has been notified of the risk.

Serious risk is not an ongoing state; it is a statutory decision based on evidence at a point in time.

The Aged Care Quality and Safety Commission will continue to monitor the performance of the service including through unannounced visits.

Following an audit we decided that this home met 30 of 44 expected outcomes of the Accreditation Standards. We decided to vary this home's accreditation period. This home is now accredited until 08 December 2019.

Serious risk decision

Following a review audit conducted on 22 January 2019 to 25 January 2019, a delegate of the Aged Care Quality and Safety Commissioner made a decision that Bupa Aged Care Australia Pty Ltd failed to meet 14 expected outcomes in the Accreditation Standards in relation to Bupa Seaforth.

On 5 March 2019 a decision was made that failure to meet one of these expected outcomes has placed the safety, health or wellbeing of a care recipient of the service at serious risk. The Department of Health has been notified of the risk.

Serious risk is not an ongoing state; it is a statutory decision based on evidence at a point in time.

The Aged Care Quality and Safety Commission will continue to monitor the performance of the service including through unannounced visits.

Accreditation decision

Following a review audit conducted on 6 September 2018 to 17 September 2018, a delegate of the CEO of the Australian Aged Care Quality Agency made a decision on 8 October 2018 to vary the period of accreditation of this service. The period of accreditation of the service will expire on 8 July 2019.

The approved provider of the service meets 10 of the 44 expected outcomes of the Accreditation Standards.

Serious risk decision

Following a review audit conducted on 6 September 2018 to 17 September 2018, a delegate of the CEO of the Quality Agency made a decision that Bupa Aged Care Australia Pty Ltd failed to meet 34 expected outcomes in the Accreditation Standards in relation to Bupa Seaforth.

On 25 October 2018 a decision was made that the failure to meet 13 of these expected outcomes has placed the safety, health or wellbeing of care recipients of the service at serious risk. The Department of Health has been notified of the risk.

Serious risk is not an ongoing state; it is a statutory decision based on evidence at a point in time.

The Aged Care Quality and Safety Commission will continue to monitor the performance of the service including through unannounced visits.

Following an audit we decided that this home met 44 of the 44 expected outcomes of the Accreditation Standards and would be accredited for three years until 20 October 2020.

This is a new home and is accredited for one year until 20 October 2017. We made the decision on 20 October 2016.
